Each page of this 4th grade science exit slip holds three copies of the same slip, so one printed sheet serves three students. The two questions check NGSS 4-PS4-1: what the amplitude of a wave measures, and what happens to a floating beach ball when a wave passes under it. A row of five faces at the bottom lets students rate how they feel about the day.
