Physical and Chemical Changes Check

5th Grade SciencePhysical and Chemical Changes2 pages

About this worksheet

One page of questions and one page of answers make up this short 5th grade check on physical and chemical changes. Students write P or C for six changes, such as cutting hair, toasting bread, folding paper and burning wood, name two signs of a chemical change, and work out the mass of salt water when 15 g of salt is mixed into 300 g of water. A final question asks why burning paper is a chemical change. Page 2 is the answer key.

Curriculum objectives covered

โ€œClassify changes as physical or chemical and name the signs of a chemical change.โ€

Items 1, 2 and 4 cover the six classifications, two signs and the reason burning paper is chemical.

โ€œAdd the masses of the parts to find the total mass after substances are mixed.โ€

Item 3 asks for 300 g + 15 g = 315 g.

What's inside this worksheet

Mark P or C

Six boxes labeled a to f: cutting hair, toasting bread, melting chocolate, rusting bike, folding paper and burning wood. Students write P or C in each.

Signs and mass

Two lines to name two signs of a chemical change, then a boxed mass question with a small picture of a 300 g beaker of water and a 15 g spoon of salt.

Burning paper

A short written answer with three lines on why burning paper is a chemical change.

Answer key

The P and C answers for a to f, a list of accepted signs, 315 g with the sum shown, and a model answer for the burning paper.

How teachers use it

End of unit check

Give it when the physical and chemical changes unit wraps up. It fits on one page, so it sits easily beside a longer test or a project.

Mark the P and C boxes first

Check items d and b first. Rusting and toasting are the two changes students are most likely to call physical.

Mass question talk

Item 3 doubles as a check that dissolving does not lose mass. Ask any student who writes less than 315 g what happened to the salt.
